If you're a new client, Dr. Johnson will set up  secure messaging with you through her secure client portal. She will send you forms to complete prior to your first appointment.

If you would like Dr. Johnson to coordinate care with another provider (for example, your psychiatrist, primary care physician, etc.), print and complete this form to authorize release of psychotherapy information:
